Yasmin Chavarin returns to our Care Coordination team!
We are pleased to welcome back Yasmin Chavarin. She joined our staff in December as a specialty care coordinator. Yasmin is already well-acquainted with our work, as she was on staff nearly five years ago as a care coordinator and also worked on our Health Homes program. She will now be focusing her time on the following specialties: Diagnostic Imaging, Neurology/Neurosurgery. We are thrilled to welcome her back.
Prior to her return, Yasmin worked as a discharge planner with the University of Washington Medical Center. There, she was responsible for helping newly released patients develop continuing care plans. She helped organize an effective post-treatment strategy for patients to heal fully at home or another care facility, carefully assessing each patient's needs for a smooth and safe transition.
When she is not providing expert-level care coordination services, Yasmin enjoys time with her young daughter. She loves to travel. No destination is too far or too close to inspire her curiosity to see the world. Her love of travel may have inspired her passion for culinary delights. As a self-described “foodie,” she tends to favor Mexican and Italian cuisines. However, recently she has found some exciting flavors and interest in the world of fusion cooking.
