This program is part of the Community Hub for both the HealthierHere and North Sound Accountable Communities of Health.

Addressing social drivers of health

The Community Resource Connections Program coordinates the work of health care and social service organizations to address health-related social needs.

We serve EVERYONE in King, Snohomish, Skagit, Island, San Juan and Whatcom counties.

These new supports include but are not limited to:

  • Comprehensive case management
  • Resource navigation
  • Referrals to meet critical health-related social needs, such as nutrition support, housing assistance, and benefits navigation (SNAP, TANF, HEN, etc.)

Eligibility

Anyone in King, Snohomish, Skagit, Island, San Juan and Whatcom counties can enroll in the program, regardless of insurance or documentation status.

How to get help

  • Complete the intake form (links below)
  • A short, confidential phone screening will be conducted by one of our Community Health Workers (CHWs) to find out more about what you need and how to best address those needs
  • Once complete, you will be referred to agencies or programs that best fit your needs
  • Your community health worker will assist you in applying for these programs to meet your goals and ensure long-term health

Program goals

The goals of the Community Resource Connections program include:

  • Expanding coverage to equitably address health care and Health-Related Social Needs (HRSNs) in King, Snohomish, Skagit, Island, San Juan and Whatcom counties
  • Advancing whole-person primary and preventative care through community collaboration
  • Supporting individuals and families in their efforts to meet HRSNs and navigate available health resources and social services
