Thanks to our recent partnership with HealthierHere and the Medicaid Transformation Project 2.0, Project Access Northwest has added a new program and expanded its team to address critical health-related social needs across King County.

Through the new program—Community Resource Connection—our team members can provide comprehensive care coordination, resource navigation, and referrals to meet critical health-related social needs, such as nutrition support, housing assistance, and benefits navigation (SNAP, TANF, HEN, etc.) to vulnerable populations across King County.

Please welcome our new Community Resource Connection team:

Vy Mai — Manager, Care Coordination

Vy Mai is our new program's Care Coordinator Manager. Vy first learned about our organization while volunteering at Seattle/King County Clinic (SKCC) and was drawn to our mission, which aligns closely with her own values.

In her role, Vy oversees the care coordinators team, connecting individuals in the community to resources that address health-related social needs beyond medical care. Her passion for creating meaningful impact is evident in her leadership and dedication.

Before joining Project Access Northwest, Vy earned her Master of Health Administration at the University of Washington and served as the executive director of Midwives’ Association of Washington State.

In her free time, Vy is an active volunteer for various organizations and serves on the boards of several nonprofits, further showcasing her commitment to community service.

Julia Harman — Care Coordinator

Julia Harman is a care coordinator for the Community Resource Connection program. Julia brings a wealth of experience to this role, having dedicated 15 years to the nonprofit sector, focusing on homeless youth and transitional housing programs. Before joining Project Access Northwest, Julia worked with the Washington State Department of Health, where she played a pivotal role in outbreak investigations related to COVID-19 and foodborne illnesses.

Julia is passionate about growing the new program and recognizes the critical need for this initiative in serving vulnerable populations. Her commitment to making a difference inspires her work as she helps connect individuals in King County to essential health and social resources.

Outside of her professional life, Julia channels her creativity through music. For nearly 15 years, she has enjoyed playing acoustic guitar and singing folk music, sharing her love for this timeless art form.

Brooklyn Switzer — Care Coordinator

Brooklynn Switzer is our newest Community Resource Connection Care Coordinator. Brooklynn was drawn to Project Access Northwest because of our commitment to serving individuals who often fall through the cracks of the health care system. She finds inspiration in our new program’s ability to help people in King County meet their basic needs, empowering them to lead healthier lives.

Brooklynn brings valuable experience to her role, having previously worked for the YMCA in Bellingham, at an elementary school dedicated to supporting lower-income families. She holds a degree in psychology from Western Washington University, which provides a strong foundation for her work in care coordination.

In her free time, Brooklynn is an avid crafter who enjoys creating handmade jewelry. Her creativity and passion for helping others make her a wonderful addition to our team.


This new team is committed to connecting individuals to resources beyond medical care, such as housing, transportation and food assistance. Their expertise and passion ensure that vulnerable populations receive holistic support. We’re excited about the positive impact they’ll make in advancing equitable access to essential care across King County!
