We’re thrilled to share some exciting news — Project Access Northwest is now part of the North Sound ACH Community Hub network!
North Sound ACH is a community-driven nonprofit transforming systems across a five-county region — Island, San Juan, Snohomish, Skagit, and Whatcom — so that everyone can get their health, social and community needs met in ways that work best for them. By connecting contractors, sharing resources, and investing in community-led solutions, they are building a future rooted in health, belonging, joy, connection and opportunity for all to thrive.
At the heart of this work is the regional care coordination hub, which receives referrals from across the region for individuals and families facing challenges related to physical, behavioral, and social needs. The Community Hub serves as a central registry for a network of care coordination agencies (CCAs), helping remove structural barriers, reduce duplication of services, and track outcomes across agencies, staff, and clients — all while supporting coordinated quality improvement efforts.
Through this new partnership, Project Access Northwest will hire new Community Health Workers (CHWs) dedicated to supporting individuals within the North Sound region who need social and health services.
“This is a tremendous opportunity for our organization to expand our reach, strengthen care coordination, and work alongside new partners to better serve our neighbors in both urban and rural communities,” says Molly James, Director, Specialty Care Coordination.
The North Sound region is diverse and expansive — from mountain ranges to the islands of the Salish Sea — and home to eight Tribal nations who have stewarded this land since time immemorial: Lummi Nation, Nooksack Tribe, Upper Skagit Tribe, Samish Indian Nation, Swinomish Indian Tribal Community, Stillaguamish Tribe of Indians, Tulalip Tribes, and Sauk-Suiattle Indian Tribe.
