Project Access Northwest is thrilled to announce our participation in the Medicaid Transformation Program 2.0, in partnership with HealthierHere. This initiative aims to enhance health care delivery and outcomes for uninsured individuals and Medicaid beneficiaries in King County.

The Medicaid Transformation Project (MTP) is Washington State's Section 1115 Medicaid demonstration waiver between the Health Care Authority (HCA) and the Centers for Medicare & Medicaid Services (CMS). MTP allows our state to create and develop projects, activities, and services that improve Washington’s healthcare system.

MTP 2.0’s theme, “Taking Action for Healthier Communities,” will accelerate care delivery and payment innovation, focusing on health-related social needs (HRSNs) and equity.

With a focus on case management, also known as care coordination, Project Access Northwest will continue to provide expert-level care and access to comprehensive healthcare services for low-income individuals and families. Through this partnership, our work will bridge gaps in whole-body care and address social drivers of health, ensuring that underserved populations receive the necessary support and resources.

HealthierHere, a key partner in this endeavor, brings together community health organizations, providers and stakeholders to create a unified approach to healthcare transformation. By leveraging their extensive network and expertise, we are poised to make a significant impact on the health and well-being of our community.
